Carotid blowout syndrome (CBS), or rupture of the carotid artery or its branches, is a sporadic complication in patients with head and neck cancer, usually associated with surgical treatment or radiation therapy. Surgical ligation of the artery has been the traditional choice to treat an ongoing bleeding from the carotid artery. Today endovascular intervention with embolization is the preferred alternative. In this report two patients are presented, with squamous cell carcinoma of the tongue and of the tonsil, respectively. After radiation therapy bleeding complication from pseudoaneurysms located in branches of the external carotid artery occurred for ballongocklusionstesth patients that were managed with transarterial embolization. Endovascular intervention is the first choice treatment for CBS. Surgical ligation should be used only if the bleeding is massive or the patients’ condition does not permits transportation to an institution with endovascular therapy resources.
