Postpartum hemorrhage (PPH) is a life-threatening condition and its most common cause is uterine atony. Immediate treatment involves arresting the bleeding, as well as communication, evaluation of breathing and circulation, monitoring, assessment and intravenous infusion of fluids. Compression of the aorta (CA) will immediately arrest life-threatening bleeding. Using ultrasound, we have been able to demonstrate the effect of CA on blood flow in a.iliaca interna before and after CA. Prior to CA, blood flow is visible in a.iliaca interna, but are no longer seen during CA. In our view, CA is a superior method of immediately arresting major obstetric hemorrhage, thus reducing the risk of coagulopathy and the need for blood transfusions. CA can be used for any major bleeding beneath the umbilical level.
