Background: Atonic postpartum haemorrhage (APPH) occurs in approximately 1% of all births and is one of the most common causes of maternal death. Atony is first and foremost treated medically, but when medical management fails, surgical intervention is necessary. In 1997, C. B-Lynch introduced a suturing technique which resulted in satisfactory haemostasis in five patients with APPH. This article presents an evaluation of the B-Lynch brace suture, based on eight cases at the University Hospital of Herlev, Denmark. Materials and methods: A retrospective study of the journal notes of all B-Lynch brace sutures performed at the University Hospital of Herlev was done. Two case histories are included in this article. The articles referred to in this study can be obtained through MEDLINE. Results: All eight patients were treated with the B-Lynch suture due to APPH following Caesarean section. In seven of the eight cases, satisfactory haemostasis was achieved after using the B-Lynch suture technique. A hysterectomy was later performed on one patient due to the vital indications. A review of all of the published articles on the B-Lynch suture technique revealed 47 successful applications out of a total of 50 (94%). Discussion: APPH can develop into a life-threatening situation in which hysterectomy may become necessary. The B-Lynch suture technique is simple and swiftly applied. In addition, this technique seems to have the same success rate as other, more complicated surgical procedures. It is therefore recommended that all doctors working in the obstetric field become familiar with the B-Lynch suture technique and that this procedure be included in the local guidelines as the surgical first choice.
