Swedish efforts to achieve improved patient safety have stepped up since 2011. New legislation became effective and the Swedish Association of Local Authorities and Regions and the Ministry of Health and Social Affairs launched a performance-based incentive scheme to stimulate patient safety work in the county councils. This paper reports from a survey among 216 people, with special responsibility for patient safety issues in Sweden’s 21 county councils. The aim was to identify the most important patient safety factors. Root cause and risk analysis was considered to have been very important to arrive at today’s level of patient safety, as well as adverse event reporting. Factors that were considered very important to attain even higher patient safety in the future concerned organizational culture, communication and education/training. Improved infection control and standardization and instruction/training regarding medical devices were also considered important issues.
