At laparotomy for acute bowel ischemia the well established principles of damage-control surgery should be followed. In selected patients with CT-verified occlusion of the SMA and without signs of peritonitis, endovascular treatment can be a favourable alternative to laparotomy and open embolectomy. Clinical judgement is still the method of choice to determine the viability of the bowel during the operation. Liberal use of second-look procedures is recommended. Abbreviated surgery include the omission of anastomoses and suturing of the abdominal wall. Temporary closure with a Bogota-bag or similar techniques reduces operating time, traumatization of the abdominal wall och reduces the risk of abdominal compartment syndrome.
