Acute bowel ischemia is a vascular emergency where bowel revascularization is necessary as soon as possible. Endovascular surgery has become the predominant treatment option with recent technical developments showing promising outcome. Laparotomy should be done liberally but can be avoided in selected cases. A “wait and see approach” to laparotomy demands intensive clinical monitoring. Optimal treatment of acute bowel ischemia requires the possibility of performing endovascular procedures in a surgical environment.
