The care of casualties has evolved during recent armed conflicts with increasingly lower mortality rates. Changes have been made both regarding pre- as well as in-hospital care. The lessons learned in the military setting can to a certain extent be applied also to the civilian EMS. Tourniquets and haemostatic dressings can be useful in certain types of injuries. Tranexamic acid should be given as soon as possible following major trauma. Prehospital use of blood and blood products has been shown to save lives, but can only be introduced on physician-staffed vehicles (i.e. Helicopter EMS). Thus every HEMS organisation should base such a decision on transport times, types of injuries seen as well as logistical considerations.
