Sammanfattning

Background: Norwegian health authorities recommend that hospitalized patients are screened for undernutrition. The aim of this study is to gain new knowledge about how clinical nurse managers experienced implementation of a screening tool for identification of nutritional risk, with a view to barriers and facilitators to implementation of the tool. Method: Qualitative study based on interviews with clinical nurse managers at two Norwegian hospitals. Results: Clinical nurse managers perceived medical nutrition therapy to be the nurses’ responsibility. Personal experience and attitudes towards nutrition were emphasized in the selection of personnel who was to perform clinical nutritional work. Barriers to using the tool included unclear responsibilities, lack of commitment and involvement from the hospital management, lack of cooperation, lack of patient influence and inadequate nutrition knowledge. Factors that facilitated use of the tool were good documentation systems, coding for the diagnosis of undernutrition, and doctors’ commitment and request for nutrition-related data. Conclusion: Clinical nurse managers may promote sound nutritional practice in hospitals through reducing barriers and strengthening facilitators to nurses’ use of the screening tool for nutritional risk. Medical nutrition therapy should be aligned with other types of hospital treatment, and roles and responsibilities need to be clarified.
