Time for new measurements in kidney disease diagnosis and follow-up
The plasma/serum level of creatinine and demonstration of the presence or absence of »proteinuria« have for decades been the basic parameters to identify and follow renal disease. These parameters have now become obsolete and should be replaced with new parameters with enhanced diagnostic performance. Among these are:
the urine albumin/creatinine ratio (ACR), informing about both reversible and irreversible glomerular damage; the urine protein HC/creatinine ratio, informing about tubular damage;
cystatin C- and creatinine-based estimate of GFR (eGFRcystatin C + kreatinin), which produces the so far best estimation of GFR with 90-95% of all estimates between +/- 30% of GFR determined by invasive, gold standard, methods for measurments of GFR.
