Sammanfattning
When establishing assertive outreach and/or integrated outpatient mental health services in Norway in 2008, the starting point was a white paper from The Norwegian Directorate of Health; IS-1554: «People with severe mental illness demanding special facilitated services». The response to this has primarily been to establish ACT-teams, or similar services with diagnosis-based admission criteria.
Baerum took another approach, wherefore a collaborative team was established with patient inclusion on 01.01.2012. Admission criteria were severe mental illness and/or severe addiction, major disability. and weak or non-existing contact with public treatment services. Diagnosis was not an assessment base for inclusion and age was irrelevant. Anyone could refer patients to the team. The project has developed a flexible, functional, committed collaboration model for primary and secondary mental health and addiction services termed ‘The C-Flex model’. We created an interdisciplinary collaboration of employees in fractional positions also staying in their initial positions.
