Sammanfattning

Older people with complex disorders and drug addiction require new collaborations across established services in the Norwegian municipalities. Separate departments of mental health and home care services are a challenge to cooperation between staff, and impede accessibility for users with complex conditions. To enhance staff collaboration and knowledge-sharing across units, we organized resource groups as learning communities of practice, in this practice development project. Each resource group is focused on the elderly’s person’s resources and challenges of everyday life, and also serves as a resource for participants. In addition, joint home visits with primary contacts across units are implemented to strengthen interaction with the user, family members, and staff. The practice development project contributed to improved understanding of the older person’s situation with complex disorders and drug addiction. Sharing experiences across departments also strengthened understanding of the regulatory framework, and how homecare and mental health can improve collaboration. Organization of collaboration requires support from health departments’ leaders to succeed.
