This report presents a case of local anesthetic intoxication caused by an injection of ropivacaine (2 mg/kg) during a surgical procedure on a foot. The injection was given about 10 minutes before the tourniquet was removed and the circulation restored. The patient developed neurological symptoms approximately 20 minutes after the injection of ropivacaine, while she was in recovery. She became unconscious, with muscular contractions and nystagmus, but recovered within minutes after an injection of intravenous lipid emulsion.
Even though local anesthetic is given within the recommended dose, symptoms of intoxication can develop under certain circumstances. Local anesthetic should not be injected into a limb until blood circulation is restored, and good knowledge of the signs of local anesthetic intoxication and of its treatment is necessary.
