Sammanfattning

Background: The prevalence of coeliac disease in children with type 1 diabetes (T1D) is between 1 % and 10 %. About 60 % -70 % of these children have no, or mild, symptoms of coeliac disease. Thus, maintaining a strict gluten-free diet (GFD) in addition to a diabetic diet might be very challenging. Objective: To present a review of national and international guidelines related to diabetes and coeliac disease in children to examine 1) whether all children with T1D should be screened regularly for coeliac disease, and 2) if a GFD is clearly recommended for children with T1D and asymptomatic coeliac disease. Methods: The guidelines were identified through searches in the Norwegian guidelines at Helsebiblioteket.no, at Guidelines International Network (GIN), and at BMJ Best Practice. The guidelines were evaluated with respect to both their quality and relevance. Consequently, four diabetes-specific guidelines and three guidelines for coeliac disease were included. Results: The guidelines provide different recommendations as to whether or not children with T1D should be screened regularly for coeliac disease. Most guidelines recommend GFD for everyone who has been diagnosed with coeliac disease, regardless of their symptoms. There is, however, an uncertainty associated with the long-term effect of a GFD for those with asymptomatic coeliac disease. Conclusion:More research relating to the long-term effect of a GFD for children with diabetes and asymptomatic coeliac disease needs to be conducted. In addition, a debate related to the frequency of screening for coeliac disease in children with T1D is needed.
