In this article, we introduce the concept of co-determination, describing its facets and how and
when it may be pursued, and set out what criteria must be met by the patient, health professional and
health system for co-determination to be a genuine option for all patients and relatives. We highlight
the benefits of co-determination and also touch on barriers. The article also presents resources that
cannot stand alone, but which serve to facilitate co-determination. We argue the case that a radical
change of culture is required in the health service for co-determination to be instilled as a focal and
natural element in good clinical practice.
