Background: More and more diseases can be prevented,
treated and alleviated through complex drug regimens. Older,
comorbid people can both get improved quality of life and prolonged
survival. Proper medication management is a key factor
for patient safety.
Purpose: This study will reveal the student›s guidance needs
by observation of the comorbid old in drug treatment. One wants
to integrate nursing knowledge with emphasis on pharmacology
in clinical situations.
Method: The project has a quality design with a follow-up qualitative
analysis of the experiences with the use of new learning
tool for medication management in home care
Results: The sample, 46 students had all an eight-week practical
training in the period 2012–2014. Students found that the
electronic documentation was inadequate. Students expressed
an increased insight into the benefits and challenges associated
with the use of various drugs. They had learned to work in a structured
and gained a greater awareness of the nurse›s expertise
relating to patient safety drug treatment
Conclusion: Using the learning tool developed in the project,
gave new knowledge to teacher, staff and students. Each
student used structure and systematics in their observations,
in addition reflection and documentation. The project
created involvement in home care. Many patients and a high
number of GPs creates challenges in the home when it came
to-date, complete diagnosis list and documented indication
for each drug.
