Background: Systematic Medication Review (SMR) is a structured
professional examination of the medicines patients
actually use so that appropriate medical treatment is ensured.
Purpose: The aim of this study is to investigate how homecare
nurses and pharmacists experience the interdisciplinary
SMR.
Method: Four focus-group interviews consisting of eight
homecare nurses and two pharmacists were performed.
Qualitative descriptive thematic analysis is used.
Results: Pharmacists and nurses emphasized three main
topic in their experience with SMR; quality in drug treatment,
vulnerable users› participation and inadequate professional
knowledge. The benefit of the SMR is good for the patient, but
there is some quality shortcomings such as lack of organizational
structure to perform and follow-up the SMR. Especially
the frail elderly are less involved.
Conclusions: SMR is a new way of working together and
involve changes in working methods both individually and
organizationally. The knowledge of each other›s disciplines
and tasks is a necessary factor to achieve new forms of collaboration
