The prevalence of substance use disorder (SUD) among patients with schizophrenia is high. The relation is bidirectional: SUD increases the risk of schizophrenia and vice versa. Co-morbid schizophrenia and SUD has important implications for both diagnosis and treatment. 4-6 weeks of abstinence from SUD is ideal, before schizophrenia can be diagnosed validly. The treatment is parallel treatment of both conditions: a combination of antipsychotic medication and non-pharmacological treatment, the latter, for example, being cognitive behavioural treatment. Prognosis is often severe.
