Hjemmesygeplejerskers rolle, opgaver og ansvar i sammenhaengende patientovergange fra hospital til primaersektor

Background: Coherent patient pathways within the health care system are challenged by transitions between sectors. The majority of cross-sectoral research is from a hospital perspective. Few studies have examined the issue from a perspective of home care nurses. However, home care nurses are pointed out as key actors in coherent patient transitions from hospital to home care. This study examines the roles, tasks and responsibilities experienced by home care nurses. Method and Results: A qualitative study based on two interviews were analysed using a Ricoeur inspired method. Three major themes were revealed: «A changed way of thinking and responsibility», «To be a security button» and «The expression of language is important». Implications for practice: Establishment of a feedback system could be an effective way to develop mutually knowledge of which factors makes the patients transition from hospital back home coherent. The Feedback systems could be made in the already existing communication system between the hospital and the home care nurses.
