Traumatic Ulcerated Granuloma with Stromal Eosinophilia (TUGSE) is a reactive process often presenting as a solitary ulcer of weeks or months duration. The borders are often indurated and the dentist frequently suspects a squamous cell carcinoma. Consequently, a biopsy is performed and the surgical procedure often induces a wound-healing response. However, TUGSE will eventually heal without surgery, but it may take several months. Histologically, an ulcerated stratified squamous epithelium with infiltration of eosinophilic granulocytes in addition to other inflammatory cells is seen. The eosinophils are characteristic for TUGSE in addition to large cells with pale nucleus and usually a nucleoli resembling macrophages, although, their origin is not clear. The inflammatory cells often infiltrate deep into the muscular tissue.
Here, we present two patients with TUGSE, an 89-year-old woman and a 64 year-old male. They both had a non-healing ulcer with indurated borders. In both cases, the surgeon suspected a squamous cell carcinoma and a biopsy was performed. The lesion healed two months after the biopsy in the first case and one month after the biopsy in the second case. During follow-up for two years and six months, respectively, no recurrence is reported. TUGSE may be more frequent than previously assumed.
