Endoscopic transoesophageal ultrasonography (EUS) gives an overview of posterior mediastinal structures and access for EUS-guided fine-needle aspiration biopsy (EUS-FNA). According to the literature, the diagnostic value of EUS-FNA in the detection of cancer in the mediastinum is high, and few complications are seen. Most of the existing studies are retrospective and include selected patients. Randomised trials are needed to evaluate the benefit of EUS-FNA in unselected lung cancer patients. The method is not suitable for visualisation of the anterior mediastinum. A new method of endoscopic transbronchial real-time ultrasound-guided biopsy (EBUS-FNA) might solve this problem.
