Sammanfattning

A recent meta-analysis has shown that neoadjuvant chemotherapy with cisplatin-containing combinations prolongs survival in patients with locally advanced bladder cancer receiving cystectomy or radiotherapy compared with a group not receiving neoadjuvant chemotherapy. The relapse treatment is, however, not well described, and chemotherapy for metastatic disease may reduce the effect of neoadjuvant chemotherapy. The disadvantage of neoadjuvant chemotherapy compared with adjuvant chemotherapy is that selection of patients is based on clinical staging. Neoadjuvant as well as adjuvant chemotherapy should be given only within the framework of a well-designed randomised trial with an untreated control group. Danish centers are including patients in an ongoing international randomised study comparing a group of patients receiving chemotherapy with cisplatin and gemcitabine immediately after cystectomy (adjuvant chemotherapy) with a control group receiving the same type of chemotherapy if a recurrence of the disease is diagnosed.
