A significant change in patterns regarding restorative caries treatment was observed in the Public Dental Service (PDS) in Oslo, Norway, 1979?-?1996. The aim of the study was to investigate if the trend towards a more defensive treatment philosophy still persisted. A representative sample (n=215) of 15-year-olds receiving dental treatment in PDS in 2004 was taken, and information on caries treatment was collected from 199 clinical records and bitewing radiographs. The data indicated that a trend towards a more defensive treatment philosophy persists. In 1996 54??% of all carious lesions in the dentine were filled, in 2004 only 30??%. The number of filled surfaces was reduced from 25??% in 1979 to 2??% in 2004. The number of deep carious lesions in the dentine was unchanged, in spite of the more stringent criteria for restorative treatment and an increase of 50??% in the mean interval between treatments. The study showed fewer sound surfaces and more carious lesions in the enamel than in 1996. The use of digital radiographs in 2004 may be of significance for this result. Supervision of the occurrence of caries among adolescents is emphasised.
