Chronic ingestion of liquorice may induce hypertension and hypokalemia. Carbohydrate restricted diets and alcohol abuse are also risk factors for hypokalemia. We describe a patient with a plasma potassium level of 1,9 mmol/l, who showed severe rhabdomyolysis after the ingestion of at least 60 g of liquorice per day for a longer period of time. The month preceding his arrival at the emergency ward, he had terminated a lifelong alcohol abuse. During this period he had also initiated a carbohydrate restricted diet. In the time period following the new diet and alcohol abstinence he had experienced an episode of muscular pain followed by dark urine. This episode occurred after an intense spinning exercise at his gym. It is important for physicians to be aware that decreases in potassium can occur after the initiation of a new diet. Moreover excessive liquorice consumption should be kept in mind as a cause for hypokalemic rhabdomyolysis.
