For more than 100 years radical prostatectomy has been an optional treatment for localized prostate cancer. Today radical prostatectomy is proven in a randomized study to reduce prostate cancer-specific mortality in localized disease. Over the years, several different surgical techniques have been developed to optimize cancer control and decrease the serious side effects that impair quality of life of men undergoing radical prostatectomy. During the last decade traditional open radical prostatectomy has been challenged by robot-assisted laparoscopic techniques. However, further advancements are warranted before the perfect radical prostatectomy will be reported. Here we review the history, surgical techniques, oncologic and functional outcomes of radical prostatectomy.
