In this article it is argued that the development of new forms of participation and involvement of the patients is a general trend in modern health care. However, at a more specific level, conceptual and practical differences and conflicts are exposed. The main focus is knowledge that patients can contribute in clinical decision making. A distinction between the knowledge of expert patients and knowledge linked to everyday life and everyday experiences is put forward. Patient-centred health care practices may be suited for independent, rational individuals as well as dependent, vulnerable persons. These different practices might conflict with each other. Furthermore, the independent, rational decision-maker is promoted at a managerial level. Therefore, it is important to stress that patient participation and patient centred health care is possible also for vulnerable and dependent patients.
