Left ventricular assist devices are implanted as bridge to transplant, bridge to recovery, bridge to decision, and as destination therapy when heart transplantation is contraindicated. Short- and long-term survival is good and number of implantations is continuously increasing in Europe and the USA. Median sternotomy is today the incision of choice for implantation. However, miniaturized left ventricular assist devices enable minimally invasive implantation, with reduced surgical trauma as a result. There is only a limited number of reports regarding minimally invasive implantation of left ventricular assist devices and it is too early to draw any conclusions regarding the potential advantages with this technique.
