Background: There is little research showing how the home
care services plan and organize visits with recipients receiving
two or more visits each day.
Method: Descriptive, retrospective method, where data
from decisions and weekly schedules from two municipalities
were collected and recorded. Geographic information systems
were used to analyze the nursing challenges related to time. It
was performed a quantitative analysis of the weekly schedules
and decisions. Geographic information systems were used to
visualize and analyze data with a geographic reference.
Result: In municipality A receives 33 % of the recipients
two or more visits per day, in the municipality B 30%. Help
for morning care, preparation of breakfast and distribution of
morning medications occurred in the period at 8:30 to 11:30.
Help for evening care, preparation of supper and distribution
of evening medication occurred between 6:00 p.m. to 10:30
p.m. The organization of the day-schedules resulted in three
recipients experienced night fasting. Several got their drugs
at intervals shorter than and some at intervals longer than
the usual dose interval.
Conclusion: Organization of visits to this group resulted in
circadian rhythm was displaced by the home care recipient. In
planning the day's schedules, there is a geographical dimension
that is not always taken into account.
Implications for practice: Employees in home care services
should be more aware of highlighting nursing challenges
related to where home care recipients live in the municipality.
