Pyoderma gangrenosum is a rare ulcerating skin disease that may not be readily recognized. It presents a diagnostic challenge
during the postoperative phase because of the similarities with postoperative wound infections. If left untreated it can lead to extensive
scarring, long hospital stays and even mortality. We describe a 56-year-old woman who developed extensive skin necrosis after a partial
mastectomy as well as extracutaneous manifestation in the gallbladder. She had a good clinical response to systemic steroids. Careful
examination of her medical history revealed several probable earlier episodes of pyoderma gangrenosum, which were undiagnosed.
Doctors should be aware of pyoderma gangrenosum as a differential diagnosis when meeting a patient with a non-healing wound and
repeatedly negative bacterial cultures.
