Sammanfattning
Living conditions, health behavior and health are unequally distributed in
Stockholm County. Primary care has an important role in detecting and providing
treatment to individuals who have a disease or are at risk of disease,
and reaching individuals who do not seek care in spite of need. Here, the
experiences from health promoting population-oriented work in primary care
from six primary health care centers in disadvantaged areas in Stockholm
County 2013-2015, are described. Lowering the thresholds for seeking care
and increasing the accessibility of care are key factors for success. In this
article, the focus is on what this way of working has meant from the perspective
of the health care staff.
